Many women choose to have their breasts enlarged in order to satisfy their own desire for a fuller bustline. Your breasts may not have developed to a size that meets your expectations, or one breast may be significantly smaller than the other. Often, after weight loss, childbirth or as a result of aging, the breasts lose volume and their shape changes. Breast augmentation can enhance your breast size and shape, restore lost self-esteem and improve the quality of life. However, you should think carefully about your reasons for having implants and be sure that breast implants are the best solution to your particular anxiety or dissatisfaction.
 
Type of mammary implants
There are two main parts to an implant: the outer shell and the inner filler. All breast implants are surrounded by a firm, silicone elastic (or elastomer) shell but contain different kinds of filler, silicone gel or saline. The surface of the implant may be smooth or textured. Breasts augmented with the textured implants are less likely to develop contracting capsules than those with the smooth implants.
 
Surgical Procedure
Placement of implant
There are two techniques for placing implants.

1. The implants are placed under the breast tissue and over the chest muscle.
The surgeon will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue or underneath your chest wall muscle (the pectoral muscle). The implants are then centered beneath your nipples.
2. The implants are placed under the chest muscle and breast tissue.
The method of inserting and positioning your implant depend on individual anatomy. Placement under the muscle however, may be painful for a few days after surgery than placement directly under the breast tissue.
The incision can be made either in the crease where the breast meets the chest, around the areola (the dark skin surrounding the nipple), or in the armpit. Every attempt will be made to assure that the resulting of scars will be almost invisible.
After surgery, stitches are used to close the incisions, which may also be taped for greater support. A gauze bandage will be applied over your breasts for two days after surgery to speed up healing.
 
Anesthesia
Breast augmentation is performed under general anesthesia in the hospital.
 

Duration of operation
About one to two hours.

 
Recovery time
You should be able to return home soon after the surgery. Often patients go home in the afternoon following surgery in the morning, or they may require just a night’s stay in the hospital. Most patients find that they can return to work after a few days’ recuperation.
 
Possible risks and complications
Capsular contracture occurs if the capsules around the implants begin to tighten and thicken. This will cause the breasts to feel hard and appear unnatural. Hard breasts can be treated in several ways. Sometimes removal of the capsule is required, or perhaps the implant needs to be replaced. Bleeding is usually minimal during the operation. In an unusual case, bleeding after surgery may cause a large blood clot that needs to be removed. Sometimes nipples become oversensitive, under sensitive, or even numb. This can notice by small patches of numbness near your incisions. These symptoms usually disappear within time.
Infection is an uncommon risk. However, if a breast becomes infected, the implant must be removed immediately. You need to wait for several months before repeating the operation.
Occasionally break or leak implants, if a saline implant breaks, the implant will deflate in a few hours and the salt water will be harmlessly absorbed by the body. If a break occurs in a gel implant, however, one of two things may occur. If the shell breaks but the scar capsule around the implant does not, there is no detect any change. If the scar also breaks or tears, especially following severe pressure, silicone gel may move into surrounding tissue. The gel may collect in the breast and cause a new scar to form around it, may require a second operation and replacement of the leaking implant. In some cases, it may not be possible to remove all of the silicone gel in the breast tissue if a rupture should occur but the new type of gel so call Highly Cohesive silicone gel will not cause this problem very often
.
 
After care
All dressings and bandages are removed on the third day following the operation. Afterwards, the patient is free to bath as usual.

The patient is requested to gently move the implants upward, downward, and inward in order to decrease the chances of capsule contracture. During the first month following surgery, you should exercise your implants three to four times per for fifteen to twenty minutes.
After that, if breast contour improve satisfactorily, you will need to exercise only once or twice daily for at least six months.There is no scientific evidence that breast augmentation increases the risk of breast cancer. However,routine mammograms should be continued after breast augmentation for who are in the appropriate age group, when request a routine mammogram, be sure to go to a radiology center where technicians are experienced in the x-ray of a breast with an implant. Additional views will be required. Ultrasound examinations may be of benefit in some cases with implants to detect breast lumps or to evaluate the implant.
