Loss of skin elasticity, gravity and other factors such as weight loss, pregnancy and breast-feeding ultimately affect the shape and firmness of your breasts. Patients who are generally satisfied with the size of their breasts can have a breast lift to raise and firm them, resulting in a more youthful breast contour. Some patients may be unhappy that they have lost a significant amount of breast volume over time. In such cases, implants inserted in conjunction with a breast lift can increase breast size at the same time as the shape and position of the breasts are enhanced.
 
Surgical procedure
Individual factors and personal preferences will determine the specific technique selected to lift your breasts.For classical mastopexy,the incision is usually made along the natural contour of your breast outlining the breast skin that is to be removed and, by doing so, the new position for the nipple is also defined. The nipple and areola are moved to a higher position after the excess skin is removed. In order to reshape the breast, the skin surrounding the nipple is then brought down and together. The stitches and resulting scar usually run around the areola and in a vertical line extending downwards form the nipple and along the lower curve of the breast.
Some patients, especially those, who have small breasts and minimal sagging, may be candidates for modified procedures requiring less extensive incisions. For example, one such procedure is the "doughnut (or concentric) mastopexy," in which circular incisions are made around the areola, and a doughnut-shaped area of skin is removed. If patients having an implant inserted along with their breast lift, the surgeon will be placed in a pocket directly under the breast tissue, or deeper, under the muscle of the chest wall.
Your surgeon will make every effort to make your scars as inconspicuous as possible. Still, it's important to remember that mastopexy scars are extensive and permanent. They often remain lumpy and red for months, then gradually become less obvious, sometimes eventually fading to thin white lines. Fortunately, the scars can usually be placed so that you can wear even low-cut tops.

About two to three hours

 

Anesthesia
General anesthesia or local anesthesia with sedation depends on any kind of surgery performed.

Hospital admission
One night admission in the hospital is required for classical mastopexy. The doughnut (or concentric) mastopexy can be performed as outpatient service.

 
Possible risks and complications
While like all surgery it does carry some uncertainty, most of the problems are minor and easily treated. Complications like bleeding and infection can cause scars to widen, though they are typically an uncommon occurrence. A breast lift may result in noticeable, permanent scars, but these are usually covered easily by your bra or bathing suit. Wider scars and poorer healing are noted in chronic smokers. Other, more infrequent side effects include unevenly positioned nipples, and loss of feeling in your nipples or breasts. Breast-feeding may not be possible after surgery.
 
After care
After surgery, patient will be wrapped in elastic bandage or a gauze dressings . A small tube may be placed in each breast to drain blood and fluids for the first day or two. The extent of the post-operative swelling and bruising is depending on whether you tend to bruise or swell easily. Applying cold compresses or ice packs will reduce swelling and relieve discomfort.
The bandages will be removed a day or two after surgery and the patient will continue wearing the bra around the clock for several weeks, until the swelling and bruising subsides. In most cases, stitches will be removed in one week.
